Part 7: Theory of Mind

Hello, and welcome back to The Autistic Writer. There are many inaccurate and harmful myths about autism and autistic people. Some of them spring from research and publications in the fields of psychology and psychiatry that have since been strongly disputed by professionals in the field.

The Theory of Mind (ToM) deficit model of autism is something most people have never heard of, although it is notorious in the autistic community.

ToM underpins certain other harmful myths about autism, like foundations support a building. Today, I will be discussing the ToM model autism, and what is wrong with it.

my story

When I first started out writing fiction, I used to seek out feedback on my works in progress from writers’ groups, and other people I could trust to be genuinely critical. As I developed, and particularly from when I completed my first full-length novel, I started getting a lot of the same feedback:

People kept commenting that the characters in my stories seemed very realistic, like real people. I received comments along the lines that I must have spent a lot of time observing the people around me.

The truth was, I hadn’t really spent significant amounts of time observing people. We all observe people all our lives. My skill was in trying to understand why people did the things they did. I’ve always been interested in what makes people tick.

what is theory of mind?

In psychology, Theory of Mind is the ability to recognise that other people have their own thoughts, feelings, beliefs, intentions, and knowledge, which may differ from our own.

In the 1980s and 1990s, psychologist Simon Baron-Cohen developed the idea that autistic people have difficulty with Theory of Mind. He called this difficulty mind-blindness. An infamous psychology experiment, the Sally-Anne false-belief experiment, was one of the key pieces of evidence supporting his theory.

His argument was not that autistic people have no emotions, but that they may struggle to infer the mental states of others in certain situations. This was presented as an explanation for many of the social and communication differences seen in autism.

The Sally-Anne test asks a child to observe an interaction between two dolls; Sally and Anne:

  • Sally puts a marble in a basket and leaves the room.
  • Anne takes the marble out of the basket and puts it into a box while Sally is away.
  • When Sally returns, the child is asked: “Where will Sally look for her marble?”

If the child answers “basket,” this is taken to show the child understands that  Sally holds a false belief because she doesn’t have all the relevant facts (the marble has been moved).

If the child answers “box”, this is taken to imply a failure to understand that Sally has her own (misinformed) mind. The conclusion from this is that the child is deficient in the capacity to understand that other people have their own thoughts, feelings, beliefs, intentions, and knowledge, which may differ from our own.

Baron-cohen’s findings

Baron-Cohen claimed the following results from his tests:

  • 85% of typically developing 4-year-olds answered correctly.
  • 86% of children with Down syndrome answered correctly.
  • Only 20% of autistic children gave the correct answer.

However, Baron-Cohen’s methods and findings have been strongly rebutted:

Major Criticisms of the Theory of Mind Deficit Model
1. Many autistic people pass the ToM tests

Baron-Cohen’s original 1985 study found that most autistic children failed the Sally-Anne false-belief task and concluded that autism involves a ToM deficit.

However, later studies found that many autistic individuals, particularly those with stronger language skills or higher cognitive abilities, successfully pass the same tasks.

For example, Francesca Happé (1994) found that many autistic participants could pass standard false-belief tests, and Nigel Bowler (1992) showed that autistic adults often passed traditional ToM tasks.

This casts doubt over both the methods used and conclusions reached by Baron-Cohen. .


2. Language ability strongly influences performance

The Sally-Anne test relies on understanding a verbal narrative and answering a verbal question. Research has repeatedly found that success on false-belief tasks is strongly associated with language development.

Work by Michael Siegal and colleagues found that deaf children with limited early language exposure showed similar delays on false-belief tasks, despite not being autistic.

Morton Gernsbacher (2007–2008) reviewed the evidence and argued that language demands explain much of the variation in ToM task performance.

The implication of these findings is that poor performance in tests of the kind used by Baron-Cohen may reflect language or communication differences rather than an inability to understand mental states.


3. Autistic people often perform better on more naturalistic tasks

Many traditional ToM experiments use artificial scenarios involving dolls, stories, or hypothetical situations. When researchers use more realistic social contexts, autistic participants frequently show greater understanding than predicted by the deficit model.

Studies by Catherine Caldwell-Harris and others found that autistic participants often understood sarcasm, irony, and non-literal language when adequate context was provided.

Research on autobiographical and real-world social reasoning has shown that autistic people can accurately infer intentions and emotions in familiar situations.

Traditional laboratory tests may underestimate autistic social understanding.


4. Failure on a test does not necessarily indicate absence of ability

Baron-Cohen interpreted failure on false-belief tasks as evidence that autistic children lacked a ToM mechanism. Critics argue this is an overly strong conclusion.

Amanda Senju et al. (2009) found that autistic adults who could explicitly solve ToM problems did not show the same anticipatory eye movements as non-autistic adults. This work is fascinating. Below is a simple summary:

What the eye-tracking task involved

Participants watched short animations in which a character places an object in location A. The character then leaves. The object is moved to location B. The character then returns.

Crucially, participants were not asked any questions. Researchers simply tracked eye movements to see where they looked first when the character returned.


What non-autistic participants did

Non-autistic adults showed a very consistent pattern. When the character returned, their eyes often moved toward the location the character falsely believed the object to be in (location A), even though they themselves knew the object had moved.


What autistic participants did

Autistic adults (including some who could pass explicit false-belief tasks) showed a different pattern. They tracked the actual location of the object (location B) instead.

However, when explicitly asked similar ToM questions in other contexts, many of these participants passed the ToM test.


How Senju et al. interpreted this

The authors concluded that autistic individuals may be able to reason about mental states explicitly, but may not automatically or spontaneously compute them during real-time social perception.

interpretation

The work by Senju et al. suggests that autistic people demonstrate differences in the automatic or spontaneous application of Theory of Mind, rather than a lack of the ability itself. This challenges the idea that autistic people lack Theory of Mind, instead suggesting variation in how and when social information is processed.

Much of the work that has challenged Baron-Cohen’s model suggests that differences in the ways autistic people communicate, interact, and process information were not fully accounted for within the original Theory of Mind deficit framework.

key point

Simon Baron-Cohen’s ToM deficit model of autism has been robustly challenged by various other professionals in the field. I would add a further, less scientific, but powerful rebuttal. You only have to spend time talking to an autistic person to know we understand that other people have thoughts and feelings different from our own. As a fiction writer, my skill in crafting realistic characters depends on this.

why it matters

The link between the Theory of Mind (ToM) deficit model of autism and the idea that autistic people “lack empathy” comes from Baron-Cohen’s equation of Theory of Mind with cognitive empathy (the ability to understand what another person is thinking or feeling). Within this idea, ToM is treated as a mechanism that allows people to interpret others’ minds.

Difficulties in false-belief tasks and other ToM measures were interpreted as evidence of reduced cognitive empathy in autism.

As this “empathy deficit” interpretation entered the public consciousness and popular culture, it was simplified into the myth that autistic people have no empathy at all.

As a result of the widely challenged Theory of Mind deficit model of autism, the non-autistic public tends to view autistic people as emotionally cold or morally detached. This misconception has fed harmful stereotypes that autistic people are dangerous, despite there being no evidence linking autism to psychopathy or a lack of emotional experience.

In reality, research and lived experience consistently show that autistic people experience deep emotional responses and concern for others, even if differences in communication style or social interpretation affect how this is expressed or perceived.
